Children under the age of 18 are not eligible to submit this form you must have your parents submit this form on your behalf. Children under the age of 18 are eligible for free lessons, provided the above guidelines are followed.

This offer is good for two separate one hour sessions of Jiu-Jitsu or Kickboxing at our Sandusky, Ohio Training facility. Attending both a Jiu-Jitsu lesson and a Kickboxing lesson on the same night constitutes two sessions. All free lessons will be held at the 1006 Tiffin Ave Training Center. Private lessons and personal training are excluded from this offer.

Once your application has been reviewed and you have been contacted by one of our instructors, you will then be given a list of training sessions that you may attend. You may be turned away and asked to return at a different time should you show up to an unapproved session.
